    Ask the Doctor: ‘What is gout, and are there any lifestyle changes that can keep attacks at bay?’

    “My father is in his early 70s and has recently moved in with me, as we did not want him living alone due to a recent fall. He suffers from gout and seems to have very bad flare-ups. I would like to be able to help him to keep this under control but I don’t really know anything about gout. My father is a very proud man and won’t let me attend the consultant appointments with him as he says he is more than capable of going alone. What exactly is gout and are there any lifestyle modifications I can introduce at home (in conjunction with him taking his prescribed medications of course) to help keep his attacks at bay?”

    Gout is the most common form of acute joint inflammation and is caused by the deposition of crystals within joints. The origin of the term ‘Gout’ stems from the Latin word ‘gutta’, which translates as ‘a drop’. It affects approximately 6% of men and 2% women. Older men and post-menopausal women are the most often affected due to hormonal factors.

    The crystals that result in gout attacks are formed and deposited when the blood concentration of Uric Acid increases beyond a point at which it is no longer soluble (>360 µmol/L). In other species, Uric Acid is metabolised to a more soluble compound which does not result in crystal formation, but humans lack the enzyme to perform this process. As Uric Acid levels rise, the risk of having gout attacks rises in parallel.

    Elevated Uric Acid can result from either over-production or under-excretion by our kidneys. Over-production of Uric Acid is typically caused by excess dietary purine, alcohol and fructose ingestion. Particular foods and beverages to avoid include those containing high quantities of fructose (fruit juices, sugary drinks, energy drinks), beer, red meats and seafood such as shellfish, sardines and anchovies. Under-excretion of gout is typically caused by chronic kidney diseases and certain medications such as diuretics, commonly referred to as “water tablets”.

    Fortunately, there are very effective medications available to lower Uric Acid, so-called Urate-Lowering Therapies (ULT). These medications work by inhibiting the production of Uric Acid which causes a fall in its concentration to the point where it again becomes soluble in blood, and so crystals do not form. The magic number to target is a Uric Acid concentration <360 µmol/L but in those with frequent flares a more aggressive strategy is advised, targeting a level <300.

    A big challenge in treating gout is the paradoxical risk of ULT precipitating flares of gout. As these medications exert their effect and start lowering the Uric Acid concentration, previously formed crystal lumps begin to dissolve and mobilise through the bloodstream, often precipitating in joints elsewhere, causing an acute painful attack of gout. This risk is present for approximately six months after commencing ULT. This generally causes a reluctance on the part of patients to take their ULT and so their gout is never optimally treated.

    To overcome this, anytime you commence ULT, ensure that your doctor also commences treatment that would normally be prescribed for an acute attack of gout. This medication should be used for six months alongside the ULT medication. After this six-month mark, it can be discontinued, and the ULT can then be safely used on its own.

    Lebanese doctor races to save the eyes of those hurt by exploding tech devices

    BEIRUT — For almost a week, ophthalmologist Elias Jaradeh has worked around the clock, trying to keep up with the flood of patients whose eyes were injured when pagers and walkie-talkies exploded en masse across Lebanon.

    He has lost track of how many eye operations he has performed in multiple hospitals, surviving on two hours of sleep before starting on the next operation. He has managed to save some patients’ sight, but many will never see again.

    “There is no doubt that what happened was extremely tragic, when you see this overwhelming number of people with eye injures arriving at the same time to the hospital, most of them young men, but also children and young women,” he told The Associated Press at a Beirut hospital this past week, struggling to hold back tears.

    Lebanese hospitals and medics were inundated after thousands of hand-held devices belonging to the Hezbollah militant group detonated simultaneously on Tuesday and Wednesday last week, killing at least 39 people. Around 3,000 more were wounded, some with life-altering disabilities. Israel is widely believed to have been behind the attack, although it has neither confirmed nor denied its involvement.

    Although the explosions appear to have targeted Hezbollah fighters, many of the victims were civilians. And many of those hurt in the attack suffered injuries to their hands, face and eyes because the devices received messages just before they detonated, so they were looking at the devices as they exploded.

    Authorities have not said how many people lost their eyes.

    Veteran and hardened Lebanese eye doctors who have dealt with the aftermath of multiple wars, civil unrest and explosions, said they have never seen anything like it.

    Jaradeh, who is also a lawmaker representing south Lebanon as a reformist, said most of the patients sent to his hospital, which specializes in ophthalmology, were young people who had significant damage to one or both eyes. He said he found plastic and metal shrapnel inside some of their eyes.

    Four years ago, a powerful blast tore through Beirut’s port, killing more than 200 people and wounding more than 6,000. That explosion, caused by the detonation of hundreds of tons of ammonium nitrates that had been stored unsafely at a port warehouse, blew out windows and doors for miles around and sent cascades of glass shards pouring onto the streets, leading to horrific injuries.

    Jaradeh also treated people hurt in the port explosion, but his experience with those wounded by the exploding pagers and walkie-talkies has been so much more intense because of the sheer volume of people with eye injuries.

    “Containing the shock after the Beirut port blast was, I believe, 48 hours while we haven’t reached the period of containing the shock now,” Jaradeh said.

    Jaradeh said he found it hard to dissociate his job as a doctor from his emotions in the operating theater.

    “No matter what they taught you (in medical school) about distancing yourself, I think in a situation like this, it is very hard when you see the sheer numbers of wounded. This is linked to a war on Lebanon and war on humanity,” Jaradeh said.

    The 1000-Lb. Sisters star, 38, posted a video via Instagram on Saturday, September 14, to share a message with her followers about losing weight.

    “Your Journey is Unique, But You Don’t Have to Do It Alone🌟,” she captioned the clip. “My journey isn’t everyone’s journey, but I know many of you have been inspired by the growth you’ve seen in me over the years. 💖”

    Slaton added that one of the “biggest game-changers” for her was getting the bariatric surgery. Her doctor, Eric Smith, has been by her side throughout the process which has been documented on the TLC series.

    Since connecting with Smith, Slaton has lost 500 pounds and she shared how proud she was of herself in an exclusive interview with Us Weekly last year.

    “The progress of getting approved for surgery wasn’t easy. I worked my butt off,” she told Us in February 2023. “I was starving a lot of the time and my hard work paid off.”

    In addition to documenting her transformation on TV, she also provided fans with updates via social media.

    “I take a lot of pictures, a lot of selfies, and I do not [take the photos] because I think I look good — well, I do, a little bit — but mostly because I want to remember how big I was, what I looked like,” Slaton said in a teaser video for season 6 of 1000-Lb. Sisters earlier this month. “And I reflect, ‘Dude, I was huge.’ I lost 500 pounds, like, I would not be able to sit in this chair.”

    Slaton has lost a significant amount of weight so far, and she’s now looking into skin removal surgery to shed excess skin.

    Cramps are irritating enough, and they can get even worse depending on your life stage. For example, many women report cramping during perimenopause, both during menstruation for those still getting periods and at other times. We reached out to our experts to find out why perimenopause cramps can be so severe and how to find relief. 

    Why do perimenopause cramps occur?

    To understand the cause of perimenopause cramps, it’s important to have a grasp on the transition itself. The National Institute on Aging describes perimenopause as the years leading up to menopause, which generally occurs 12 months after a woman’s last period. The years leading up to that point, when women may have changes in their monthly cycles, hot flashes or a variety of other potential symptoms, are called the menopausal transition or perimenopause.

    “People who have never experienced cramping may now experience cramping during [perimenopause],” explains Rajita G. Patil, MD, Obstetrician-gynecologist, Director – Comprehensive Menopause Care Program at UCLA Health. “Others just have it worse. This is all because of fluctuating hormone levels that occur during the transition to menopause. The periods during perimenopause become unpredictable in timing and they may come earlier or later and eventually, period cycles can be skipped.” 

    The normal fluctuation of estrogen and progesterone does not occur anymore once the body is in perimenopause, she says, so the estrogen levels in the body are at times higher than what would be normally present during a natural cycle during reproductive years. This causes the body to release more prostaglandins which cause more contractions of the uterus, which lead to more cramping. 

    How to treat perimenopause cramps

    There are a few different options for relieving cramp pains including over-the-counter products, birth control and natural remedies. 

    Pharmaceuticals 

    “Some of the best treatment options are nonsteroidal anti-inflammatory medications (NSAIDS) like ibuprofen which can really help with cramping pain,” offers Dr. Patil. “You want to use them right when you start having the cramps and not when they are already very severe to help stay in front of the pain and not behind it.”

    Hormonal birth control can also help regulate the cycle and keep the estrogen levels in check to minimize cramping, she adds. Hormonal birth control also limits the amount of uterine lining that is built up each month that needs to be shed with the period, which in turn helps to minimize the number of uterine contractions that are needed to shed the lining which decreases cramps. 

    “A hormonal intrauterine device (IUD) can especially help control the pain by limiting the local estrogen effect in the uterus,” offers Dr. Patil. “It is less systemic in nature than most of the other hormonal birth control methods like the patch, pills and vaginal ring.”

    Diet and exercise 

    Another non-pharmacologic treatment that helps reduce pain is moderate intensity exercise as it helps to divert blood flow to the brain and help release endorphins, she notes. This can help decrease the perception of pain from the cramping.

    Additionally, applying heating pads to your lower abdomen may help soothe some of the pain, and mind-body relaxation techniques can be an effective coping strategy. These practices can help manage stress levels, and one study published in the journal Occupational & Environmental Medicine found that period pain was twice as common in women who were stressed out than in women with low stress. 

    Avoiding caffeine and spicy or greasy foods can help prevent additional lower abdominal discomfort during uterine cramps as well, Dr. Patil says, as these foods may irritate the bowels.

    Should you see a doctor for perimenopause cramps?

    Cramping during perimenopause is not always exclusively caused by hormone levels, so at a certain point, you’ll want to see your doctor to rule out more serious medical conditions. 

    “You should see a doctor if the pain is not well controlled with lifestyle measures described above or use of NSAIDS and/or if you feel the pain is not controlled enough for you to go about with your normal day,” says Dr. Patil. “It would be important to further discuss what treatment may better relieve your symptoms. Also, because cramps are not always because of perimenopause and changes in the hormone levels, it is also important to seek help if pain is worsening and/or associated with any abnormal bleeding outside of your period cycles.”
